Annual or Life Membership Application Form

If you are a Purple Heart recipient (male or female), please use this form to join the Military Order of the Purple Heart and our Chapter 353 as an Annual or Life Member. You can also use it to renew your Annual Membership.

Associate Membership Application Form

If you are a male parent or lineal descendent of a Purple Heart recipient, we invite you to join the MOPH and our Chapter as an Annual or Life Associate Member. You can also use this form to renew your Annual Associate Membership.

Membership Transfer Request Form

If you belong to a MOPH Chapter or are a National/Department Member-At-Large, please use this form to request transfer to a Chapter of your choice, including our Greater Washington Chapter 353.
